Custom Question(s)/Additional Question in "Fields" for Appointment Type

We would like the ability to add a custom question or a fixed question when the customer is scheduling their TimeZest appointment:
"Would you like us to contact you earlier than your scheduled time if an earlier appointment becomes available?
Option to answer "Yes" or "No"

We currently use Acuity for our bookings, but we need to integrate it with Zapier to transfer information from the booking itself into CW PSA.
The reason we use Acuity is that we can create custom question types, such as text answers, radio buttons, and drop-downs.
If this functionality were added to TimeZest to allow us to add additional questions to the booking that are then added to the ticket description upon booking, we could move away from Acuity/Zapier and use TimeZest.

We use TimeZest for onsite appointments for prospects that are not in our PSA. The ability to have and require an address field for an appointment type would be helpful so they can input their address when scheduling with us.

This would be a great feature - Having the ability to Request our subcontractors to acknowledge that they have and will bring specific tools (i.e. Laptop, specific length ladder, ect.) would be invaluable

We asked for an extra field in a previous request. We need to collect serial number and store the info in the ticket notes.
